Active Ingredients:

  • Sitagliptin Phosphate 50 mg (DPP-4 Inhibitor)
  • Metformin Hydrochloride (Extended-Release) 500 mg (Biguanide class)

Therapeutic Class:

  • Antidiabetic agent (Combination of DPP-4 inhibitor and biguanide)

Indication:

  • Janumet XR is used to improve glycemic control in type 2 diabetes mellitus in adults.
  • It is used as an adjunct to diet and exercise in patients whose blood glucose is not controlled adequately with metformin or sitagliptin alone.
  • Helps lower blood glucose levels by improving insulin sensitivity and reducing glucose production in the liver.

Mechanism of Action:

  1. Sitagliptin (DPP-4 Inhibitor):
    • Inhibits DPP-4 (dipeptidyl peptidase-4) enzyme, which breaks down incretin hormones.
    • Incretins enhance insulin secretion and reduce glucagon release in a glucose-dependent manner, helping to control blood sugar levels.
  2. Metformin (Biguanide):

    • Reduces hepatic glucose production.
    • Increases insulin sensitivity in muscle and fat tissues, improving glucose uptake.
    • Delays the absorption of glucose from the gastrointestinal tract.

Dosage & Administration:

  • Taken once daily with food to minimize gastrointestinal side effects (preferably with the evening meal).
  • Do not crush or chew the tablet; it should be swallowed whole.
  • The dose is individualized based on blood glucose levels and patient tolerance.

Contraindications:

  • Severe renal impairment (eGFR < 30 mL/min/1.73m²)
  • Metabolic acidosis (including diabetic ketoacidosis)
  • Hypersensitivity to sitagliptin, metformin, or any excipients
  • Pregnancy and breastfeeding (unless prescribed by a doctor)

Warnings & Precautions:

  • Renal function must be monitored before and during treatment.
  • Risk of lactic acidosis in patients with renal or hepatic impairment, especially if metformin is used inappropriately.
  • Hypoglycemia may occur when combined with other antihyperglycemic agents like insulin or sulfonylureas.
  • Avoid alcohol consumption, as it increases the risk of lactic acidosis and hypoglycemia.
