Synjardy 5 mg/850 mg Film-Coated Tablets (60's)

Active Ingredients:

  • Empagliflozin 5 mg (SGLT2 inhibitor)
  • Metformin Hydrochloride 850 mg (Biguanide class)

Therapeutic Class:

  • Antidiabetic agent (Combination of SGLT2 inhibitor and biguanide)

Indication:

  • Used for the management of type 2 diabetes mellitus in adults.
  • Helps improve glycemic control when diet and exercise alone are insufficient.
  • Lowers blood sugar by blocking glucose reabsorption in the kidneys and reducing glucose production in the liver.
  • Provides additional benefits such as weight management and cardiovascular protection in patients at risk.

Mechanism of Action:

  1. Empagliflozin (SGLT2 inhibitor):

    • Inhibits sodium-glucose cotransporter-2 (SGLT2) in the kidneys.
    • Increases urinary glucose excretion, leading to lower blood sugar levels.
    • Reduces glucose toxicity and insulin resistance.
  2. Metformin (Biguanide):

    • Reduces hepatic glucose production.
    • Improves insulin sensitivity in peripheral tissues.
    • Slows down intestinal glucose absorption.

Dosage & Administration:

  • Taken once or twice daily with meals to minimize gastrointestinal discomfort.
  • Tablets must be swallowed whole; do not crush, split, or chew.
  • Dose adjustments may be required based on renal function and patient tolerance.

Contraindications:

  • Severe renal impairment (eGFR < 30 mL/min/1.73m²)
  • Metabolic acidosis (including diabetic ketoacidosis)
  • Hypersensitivity to empagliflozin, metformin, or any excipients

Warnings & Precautions:

  • Kidney function should be assessed before and during treatment.
  • Risk of lactic acidosis, especially in renal or hepatic impairment.
  • May cause hypotension due to fluid loss, particularly in elderly patients or those on diuretics.
  • Increased risk of genital and urinary tract infections due to glucosuria.
  • Should be avoided during pregnancy and breastfeeding, unless prescribed by a doctor.

Common Side Effects:

  • Gastrointestinal discomfort (nausea, diarrhea, abdominal pain)
  • Increased urination (polyuria)
  • Mild dehydration or dizziness
  • Genital infections (e.g., yeast infections, balanitis)
